I have been following with interest the thread re the Gail model. Yesterday I saw a pt that I would like to present for your advice. 43 yo G2P2. Regular cycles. Menarche @ 13. 1st del @ 31. Mother diagnosed with breast cancer @ 75. (Mothers genetic BrCa is neg). Pt has had 2 biopsies, one with atypia. This puts her at 7.4% 5 yr risk and 47.5 lifetime!!! Patient then commented that she was not confident that her biopsy was properly diagnosed with atypia. Therefore I redid the Gail model and that put her at 3.8 / 28.5!!! I think I kept my composure in the presence of the patient, but didn't sleep well last night! What advice would you give this woman? Thank you for your input.
