Some years ago I had a similar case. Patient in early 20s status post bilateral oopherectomy for large dermoid cysts. I changed her from Premarin (.625) and Provera to OCs because it seemed to me that a young women needed a higher dose of estrogen. However, I could not find any justification for my decision in the literature.

If you find any information, please post it.
