I do offer enrollment in the STAR study...luckily there is a trial site nearby. I guess the question is, if you order a test, what are you going to do with the results? Since the Gail calculation is a test, if you aren't going to follow the NCI recommendations, why do the study at all? If on the other hand the patient understands the rational for the Gail Model calculation and decides to abandon the recommended prophylactic treatment option (Tamoxifen) or enroll in the STAR trial, I probably would keep her on HRT, with a heighten awareness and screening for breast cancer.

All my patients 50 and over are suggested to get colonoscopy. I offer to set up the appointment, just like with mammograms or other diagnostic studies. That way I have been proactive and it is the patient that opts to decline the study.
