You're right about the lack of integration. This patient lives in a community about 75 miles from here with a small community. When she went past 42 weeks, the lay midwife wanted to have her go to the hospital for a NST and contacted all of the GPs doing Ob in the community, non would approve a NST - because "she's not my patient." She even contacted the ER physician, who refused to get involved. I'm not sure if she physicically went into the ER and was sent on her merry way, or if it was just a phone contact. If it was a physical presence in the ER and the patient was not adequately evaluated, in my mind that is below the standard of care, and probably violates the COBRA statutes. She ended up in my town because the lay midwife is friends with one of the CNMs I back-up.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
