I've done 2 in 2 years in private practice (both in the first 6 months...). One was for back down transverse lie and the second was a 26 week delivery with basically no lower uterine segment to incise.

Both are truly more low verticals than true classicals (neither extended to the uterine fundus) but in the end it makes no difference in my mind.
