To cerclage or not to cerclage that is the question

Consider the case of a 19 yo primip with 17+ week twin pregnancy who was referred to a perinatologist for a level II US. The patient is asymptomatic. The perinatologist calls and tells you that the anatomical survey was normal but the cervical length is 2.2 cm and the lower uterine segment is bulging. She recommends a cerclage. The patient is asking for your opinion.
