Our MFM consultant would (I think):

1. Admit to bedrest/trendelenberg or at least head down 2. Monitor for contractions, low threshhold to start Mag Sulfate and/or Indocin (short course) 3. If stable overnight, and not contracting, perform rescue cerclage with large foley balloon under sonar guidance to reduce the membranes (yes, I know that they are not external to the os--put the foley through, and inflate the bulb, and then stitch it, deflate and pull the balloon. 4. Antibiotics for a week or two
