New confusing caseFrom: amarnath (amarnath@bom2.vsnl.net.in)Sun Mar 12 05:53:09 2000
Dear listmembers, I saw new case and posting iton the forum with the consent. 16 yr old unmarried girl, sexually not active, brought by her mother with complaints of secondary amamenorrhoea. FMP at the age of 14 years when she had a period just once. This was a regular period, and not just spotting. No other complaints. On examination, ht-1.62M, wt-53 kg, BMI of 20.2, breasts B5, sexual hair P5. The girl is physically fit, scholastically doing very well. No food fads. Investigations were as follows - sr E2 - 75 pg/ml Sr. P - 0.3 ng/ml (anovulatory) Sr. FSH - 6.2 mU/ml Sr. LH - 5.1 mU/ml S. PRL - 11.1 ng/ml Sr. Tsh 1.72 microU/ml (WNL) USG showed a normal uterus, and both ovaries were seen to be normal. As the mother was getting too worked up, I decided to document a peiod by giving her T. Premarin 0.625 mg QDS ( yes, 4 times a day) for 20 days with T. MPA 10 mg daily for the last 5 days. To my surprize, there was no withdrawal bleed. She completed the course on the 20th of February. What am I dealing with? EM TB? how do I document it? could there be any other cause? Am I missing something? Thanks for your help. Dr. Amarnath Bhide Professor of Obs-Gyn. N. Wadia Maternity Hospital, Mumbai, India

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
