The first C/S I ever participated in when learning to circulate as a new nurse was an anesthesia fiasco. The section was initiated under local after a failed epidural, then failure to intubate following general anesthesia induction sequence. The mom was hypoxic, the baby bradycardic, and the section was begun under local while the anesthesiologists (3 of them) struggled to get her intubated, ventilated, and anesthetized. It was a train wreck.

Hard saying how much that poor woman felt, as she was already immobilized. She aspirated blood and gastric contents and spent several days in the ICU. The baby was in the NICU for quite a while. Though term, he had necrotizing enterocolitis from asphyxia.

Not exactly the same sort of scenario the rest of you are reporting for why you had to do C/S under local, but unusual events clearly do call for extreme measures on occasion.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
