I know one of the authors. I'm not convinced that they have go the balance right. They failed to mention the issue of steroids in twins and didn't consider the fact that one might have given prophylaxis weeks ago and the babe wasn't delivered and now the mum is in preterm labour/has severe preeclmapsia/etc.

Personally I'd give a second course if time allowed. The evidence is probably against multiple course - not two courses weeks apart!
