My opinion is that this patient is best advised not to think about attempting a vaginal delivery. For two reasons: 1. She has a uterus with three scars increasing her risk of rupture 2. The pelvimetry is well outside the guidelines that I would regard as indicating an adequate pelvis. There have been studies showing that the chance of vaginal delivery is markedly reduced with an inlet diameter of <10cm. And the subpubic angle is also too small. The presence of an occipitoposterior in the first pregnancy is a warning to me too.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
