I would do a vaginal ultrasound and biopsy with a pipelle if thickened endometrium (>5 mm). If non-diagnostic, I would do a hysteroscopy. Another option (if money is a major concern) would be to give 10-12 days of provera, if no withdrawal bleed, estrogen primed endometrium would be pretty much ruled out and proliferative or hyperplastic endometrium would be unlikely, but I don't think it would completely rule out a polyp. If she withdraws, I would go back to plan A, sono and bx.
