If you have a totally demented patient you can't do anything under local! My recent case was so nutty that she didn't understand that she was in hospital. We got her undressed and into a theatre gown, but she got dressed ready to go home. We walked her to theatre where she had an inhalational induuction of GA, whilst sitting in a chair (ordinary desk type chair), wearing her corsets, underwear several layers of clothes and an overcoat and hat. Her handbag was in her hands on her lap. Once asleep I lifted her onto the "gurney" with the anaesthetic assistant and the nursing staff undressed.

The vaginal hysterectomy and anterior repair did take over 15-20 minutes (just) but that was because I let my resident do the surgery under my supervision!
