If you have a totally demented patient you can't do anything under local! My recent case was so nutty that she didn't understand that she was in hospital. We got her undressed and into a theatre gown, but she got dressed ready to go home. We walked her to theatre where she had an inhalational induuction of GA, whilst sitting in a chair (ordinary desk type chair), wearing her corsets, underwear several layers of clothes and an overcoat and hat. Her handbag was in her hands on her lap. Once asleep I lifted her onto the "gurney" with the anaesthetic assistant and the nursing staff undressed. The vaginal hysterectomy and anterior repair did take over 15-20 minutes (just) but that was because I let my resident do the surgery under my supervision!

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
