LWM heparin

at what stage of pregnancy would you start LMW heparin if woman w/ +Factor V Leiden, etc? Assuming that the diagnosis was made based on poor ob hx (habitual ab) would you start at dx of pregnancy, confirmation of viability, 2nd trimester?
