Like most US obstetricians, I have my patients start their pills on the sunday after their menses begin, for the reasons already stated: conforming to the packaging, simplicity and uniformity. Speroff states that starting oral contraception on the first day of menses assures immediate protection, but also acknowledges if they are started no later than the 5th day of the cycle, effective contraception is present and that if a dominant follicle should emerge after a sunday start, an LH surge and ovulation will still be prevented. >
