Hi Erin, I'm not Dr. M :-) but I can answer this from my experience. A rectal exam can hurt alot because we tense up, are not relaxed and when something is inserted anally under these conditions, it hurts! In order for us to have a painless rectal exam, we have to be able to relax the anus. That's usually why they tell us to take a deep breath and relax. Some people have certain ideas or notions about the anus and it's just not yet possible for them to relax it mentally or physically. It also relates to whether you expect it to hurt--if you do it will. Just my 2 cents :) Gina
