Your concerns are valid and I'm glad you asked. Hopefully others can give you answers that I can not.

I have had IBS since my teens. I have also delivered 3 healthy children. For me it was a matter of isolating the triggers. Triggers can be foods, food additives (chemicals), dyes or stress, and possibly more. For me personally the prenatal vitamins irritated my IBS, infact many multivitamins still do. It seems that our triggers are all very much a personal thing, no two people are alike. As for the pain I try to relax and if possible nap. If I get tense/stressed it seems to get worse.
