The diaphragm has an 84% efficacy per woman year, so failure is relatively common, and a retroverted uterus makes it more difficult to get the diaphragm in the right position over the cervical os, so failure is more common. Nursing can reduce the number of ovulations, but that number is rarely zero. Sorry.
