Sorry to hear about your problem. While no one can make the decision for you - if possible, get as many opinions as possible and find the doctor and the procedure that best suits you. If possible, see if you can find a urogynocolist to examine you (standing). This seems to be rather a new specialty, and while the art of pelvic reconstruction may not be new to some medical professions, it now has a name. If you decide to go the surgical route, try to find a very qualified doctor versed in the most current procedures. The sling is used a lot for UTI, but the Burch seems to have a much higher rate of success.

As for a hysterectomy, I can only say that for myself, I don't think all the organs in a woman's body are fully understood - there are a lot of subtle functions I think that most are unaware of (like the brain, unchartered territory). My thinking is that once the uterus is out, that's that - I'd try to preserve what I can before I go that route.
