How to tell if pain is from trapped nerve and/or adhesions. You can't.

Granulation tissue is not an adhesion. It usually doesn't hurt.

Adhesions of themselves usually do not hurt. It is only when the pull on, or, obstruct an organ that the organ cause the pain.

Looking with Lapscope might give you an answer. OR - - carefully opening p the top of the vagina could also help. (This must be done very carefully because bowel, etc., may be stuck to top of vagina.) Lapscope is safer. Open abdominal procedure may be needed to free you from pain.
