We have around 60 babies born per month and very rarely run into complications that would require an NICU. Most small hospitals are located close enough to large hospitals with NICU's. In addition I believe that all hospitals have to have people on duty that have to be certified in neonatal advanced life support. As for babies being jaundiced, the way that it works at our hospital is when babies are readmitted for phototherapy they put the babies in a regular room (and the mom can stay in the room with them) and the nursery nurses and the regular labor and delivery nurses take care of them. It is a policy in my hospital that all babies under 1 month are taken care of by the nursery nurses. I hope this helps.
