I think that your form looks great! The only comments I would make if you were one of my clients is that it is very long. Most patients will not read that many questions which will in turn prevent you from getting all of the information desired. We have found that patients are more likely to answer the questions if a nurse asks them. It does take more time this way, but you will have accurate responses. The other thing that we have recommended is having the patient fill out the form, but then have the nurse review it prior to the doctor going into the room.
