hi,it could be both.it sounds like you may have some hemorrhoids too.also if you're assisting you may be scratching the rectal vault,do you use a lubricant? i suggest increasing your fluid intake-one of the best things to help with constipation along with increasing activity.maybe a mild stool softener would help too.how long has this been going on?i also suggest seeing a dr.
