Sometimes it requires diverting the bowel contents (a colostomy) so the tissues around the fistula become less inflamed and will heal better after a repair. Often, a surgeon very experienced in repairing rectovaginal fistulas may be needed if hte fistula is large or has not responded to previous treatment. You should talk to your doctor about what your options are. You may want to ask for a referral to a gynecologist who has done additional training in pelvic reconstructive surgery.
