Once you have a ++ test, you DO NOT need to continue testing. LH will remain above the test threshold, and give you a ++ test for several days -- once it turns ++ you don't need to waste your money by doing further tests. It says that in the instructions of all the test kits I've ever seen (or used myself).

Yes.

Other than what I wrote above or pregnancy, I can't think of any off the top of my head.
