I'm a 34 yo para 4, grava 2, with endometriosis, tipped uterus, irregular menses & dysmenorrhea, raging hormorne storms, seemingly relentless HA'S and fibroids. In '86, a laporoscopy was performed to evaluate for endometriosis= positive, no therapy performed. In '95 & "97 the second and third procedures were performed= confirmed endo and still no therapy due to its wide spread on the abdominal contents including the liver. Folowing the '95 procedure, a two dose trial of Lupron was completed with marked success. I felt great. Unfortunately, the therapeutic quality did not last. Now that the latest procedure has revealed what I already knew to be present, my OB has recommended a total (including the ovaries) hyst. My husband watched a LAVH on the Learning Channel, and feels that regardless if the ovaries are taken or not, this should be the procedure of choice. My OB stopped doing them awhile ago because of the prospects of retained staples causing SBO. Mother has had breast & cervical CA, Grandmother uterine CA. Considering the HA's, wild mood swings and dysmennorhea, I'm heavily leaning towards the prophylactic removal of the ovaries, however am greatly concerned with the prospects of depression considering an eight month bought with mental illness following my second child's birth six years ago (am fully recovered now). Question 1: Concerning hormone therapy, how does one know what the therapeutic dose is? What is too little or too much? I'm concerned with a dry vagina. What about weight gain? Question 2: If the ovaries are spared, won't the presence of the remaining endometrial tissue in the abdominal cavity still proliferate and sluff-off, causing the terrible cramping each month? Question 3: Are there any contraindications to the LAVH technique specifically related to my tipped uterus and endometriosis? Question 4: If the ovaries are spared, does the ovary still release an egg, and if so, where does it go once the uterus is gone? Question 5: Have heard of embolization therapy of the feeders to the fibroids. Considering this isn't my only delimma, should this be a viable consideration. Question 6: Depression. Is it really a concern? I realize that I've placed alot of informaiton, however feel that I'm certainly not the only one out there to ponder these questions.
