If you know what city he is in, you can call directory assistance and see if there is a listing for his office (AT&T has a number where you don't need to know the area code 1-900-555-1212 and they will look it up for like 50 cents.) All else fails, write to the address you have, and tell him what you want. He can always pass it to his office manager or nurse for handling.
