PREGNANCY: metformin and menstrual cycles

Hello, I am wanting to get pregnant but have a history of endometriosis, suspected adenomyosis, PCOS, and removal of left ovary and tube. I was on depo provera for several years and quit taking it approximately 2 1/2 years ago. I had normal menstrual cycles (about every 23 days) for about a year, then had a lap in June, 2008 and had my ovary & tube removed due to an endometrioma and endometriosis. I had a menstrual period every 23 days after that until October (each month had a spike in temp and positive ovulation sticks)...then, no period until January, 2009. Had an HSG after my period in January, had some bleeding for a few days after that. The bleeding stopped, then about a week later, started again (early for my cycle). It lasted until end of February when my doctor gave me provera to stop the bleeding. Took it for 10 days, then nothing for 14 days, then provera for another 10 days. Had spotting for a day in March. During that time, I was seen in ER because the pain was so bad....they said it was just the endometriosis and cramps...no infection. I have not had a period since then. June 5th, I started another 5 days of provera. Still no period. Lots of cramping....no bleeding. I also started taking Metformin in April. Could this be a side effect of Metformin? The last FSH I had (in January) was 9.5. I am scared this means I'm going through menopause...I don't know what else to do to make my periods start again. Is this how menopause works? I am 41 years old, no children, had a previous laparoscopy to remove an endometrioma from the right ovary but they were able to save the ovary at that time. The HSG I had showed no blockages. Any advice?
