You may want to bring this up to the doctor BEFORE your next examination. Maybe you can take a pain killer before the examination or ask for a smaller size speculum to be used, etc. If you don't tell the doctor what is going on BEFORE the examination he/she will not know it.

Also, during the exam if you feel anything let the doctor know! Sometimes it is the doctor - what I mean is I have had examinations done with different doctors and some are more gentle/do it better than others.
