PREGNANCY: Pregnancy Kits and its accuracy at ob/gyn

My fiance visited a ob/gyn some time last year and got tested to see if she was pregnant but the the test came back negative from the urine test. Before she vistied we've tested the pregnancy test kit at home and came back negative. So we both assumed that she was not but about 2 weeks after we visited another doctor and did one of those sonograms and it returned that she was 6 weeks into pregnancy. I was wondering is it possible for a person to be unable to to determine pregnacy by urine samples? and that perhaps sonograms is the only thing that can determine her pregnancy?
