PREGNANCY: Miscarriage at 13 weeks

Last year I had a miscarriage at 13 weeks 3 days. I have just gotten the medical records from the D&C I had. I'm really confused. I had an US in my ob's office at 8 weeks and 12 weeks 6 days. Both times a heartbeat was confirmed. We saw the heartbeat but did not hear it as his machine did not have sound. I measured right on and labs were normal. At 13 weeks 3 days, I had a peri appt for the NT scan. At that appt, the US showed no heartbeat. It was confirmed. I was told the baby had died in the last day or two as the baby measured 13 weeks. On the US all 3 times was a baby. I can tell you exactly how the baby looked on the last US even though I did not get a picture. There was definitely a head, arms, legs, ribs, etc. Just no heartbeat. I had a D&C immediately the next day. My operative report says that the baby was a blighted ovum, intrauterine fetal demise at 13 weeks. The pathology report showed Uterine Contents: Decidua and Chorionic Villi identified. Then fetal demise at 13 weeks. It says that reddish hemmorrhagic soft tissue was submitted. So where I am confused at is was this an early pregnancy loss that was missed or was the baby really 13 weeks gestation. I don't understand how the baby could be classified as a blighted ovum and we saw a baby on the screen three times & a heartbeat twice. The US tech at my peri's said the baby measured 13 weeks. After the D&C, my uterus filled with blood, clots, and debris. I got a uterine infection. The decision was to wait it out as another d&c could cause the same to happen. 2 weeks later, I spent 2 days tied to the bathroom because of bleeding and contractions. I eventually got rid of an enormous clot bigger than my hand. After that the bleeding stopped. Another question was if the baby was really 13 weeks gestation, is it possible that my ob did not get the baby and what happened those two days was me passing the baby? I was in terrible pain. In fact, I called my ob and he called in lortab. I guess I am just surprised for a baby that passed in a day or two of the US was just tissue and not the baby we saw on the screen. Unfortunately we did not get pictures. My ob's machine is old. I was so upset the day at my peri's I didn't think to ask for one. I have since had another baby. I am thinking about another one in 2 years. Knowing what this says now, how can I ever trust another US again. Who is wrong? The reports or the US? I would just like to know what happened. Thank You.
