D, I'm not an expert on rv fistulas, but have read about them in the archives here, plus some other articles. (doing a search in google should bring up several leads). A birth injury is a common way, but there are all sorts of ways to develop them: surgery, non-surgical related infections, some types of diseases. You'd need to see a doctor for some tests to rule out fistula. I'm not sure about the shooting pains- hopefully a doc will come along soon and expound on some other possibilities. I had shooting pains in the vagina, pelvis and rectum for many years before I had a hysterectomy due to numerous large and impacted fibroids which also caused bladder problems. (all behind me now, thank goodness!) Call your gyn for an appointment- I hope you get to the bottom of this soon and feel better.
