Both ultrasounds missed it?

I recently was sent off base for a pelvic ultrasound (they suspected a mass still there from a hysterectomy done last year). During the ultrasound, they weren't able to find one of my ovaries, so they also did a transvaginal ultrsaound...and were still unable to locate it. I know that an ultrasound can miss an ovary for one reason or another...but I was wondering why they both missed it.

I'm a little concerned, because I do have alot of breast/colon cancer in my family and had recently been experiencing abdominal bloating/hardness, fatigue (of course that could be due to labs done in December that I just got the results from this month, showing highly anemic), bleeding after intercourse, slight sharp pains in lower abdomen, and changes in stool..I was curious as to whether it should be followed up or if it's fairly normal for both tests to miss the ovary.
