Why a Dermoid Diagnosis?From: Simone (anonymous@obgyn.net)Thu, 28 Feb 2002 11:05:14 -0600 (CST)
Dr., I am 38 years old, have never given birth and as far as I know, am in good health. I visited my gyn. last week because I believe I'm experiencing symptoms of perimenopause; heart palpitations, bloating and night sweats. She said I was too young but ordered a pelvic US with transvaginal follow-up to check the lining of my uterus. The US revealed a .57 cm. calcification in my left ovary (also a fibroid which I knew I had). My doctor believes the calcificationis indicative of a dermoid tumor and has scheduled a diagnostic laparoscopy with possible cystectomy or removal of part of the ovary. Below is the US report, however my questions are 1) why a dermoid if there is no mention of a "mass" shown in the ovary? 2) If there is a dermoid present, wouldn't my left ovary measure larger than the right? 3) Do the measurements noted seem within normal range? I realize this is a bit long and I apologize for that. Any insight you could provide would be greatly appreciated. REPORT: Multiple sections were obtained which revealed the uterus to measure 8.90 x 4.22 x 5.13 cm. There is a small 2.02 x 1.86 x 2.41 cm fibroid anteriorly. The right ovary measures 3.74 x 2.42 x 3.21 cm and the left ovary measures 3.11 x 2.76 x 2.29 cm. It contains a .57 cm calcification within it. No fluid is identified in the pelvis. IMPRESSION: 1. Small anterior fibroid which has increased slightly from 8/2000 2. A 5.7 mm calcification in the left ovary which vaguely was defined 8/2000 but larger on the current examination. A neoplastic process here not excluded.
