Pre Menstrual Spotting and early periods

I am almost 40 years old. I have endometriosis, fibroid tumors,and cysts on both ovaries.None of thes conditions gave me problems until the last few years. It started with bleeding after sex, that would come and go. I had a pap done, a colposcopy with endocutterage biopsy,and several ultrasounds.It all came back fine.Then after about a year, my cycles started to shorten. I would get my period on the 23 day instead of the 28 day. I spot for a couple days before my period really kicks in. I bleed for a total of seven to nine days, including the spotting days. A couple of months, I spotted around the time of my ovulation.Then I will have a month when I do not spot at all. I have been having ultrasounds done every two mojnths to moniter my cysts. My doc also did a Hysterosonogram to rule out uterine polyps,my lining was normal,eveything looked o.k. I am worried,of course about cancer. My Doc feels she has done all the tests to rule this out.I asked her about an endo biopsy,and she feels Indo not need one,and that they can be inconclusive anyway.She is going by the Hystero sonogram results,she seems to feel that is a reliable test.Could this all be the start of menopause? Can the endo be causing the pre menstrual spotting? Should I have a D=C to biopsy and rule out uterine cancer? My doc feels she has tested me for everything,and she thinks it is unlikely that cancer is causing these problems.I would appreciate any input on this matter. Thanks Alice
