Phone in Provera or exam first?

I'm curious. I've missed two cycles and I know I'm not pregnant (celibate for several months now). I went for a gyn exam a few months ago and all was fine. I'm in my 30's and have no health problems. Here are my questions:

1. I've read about Provera being used to start a period. Is this something my dr could just phone in or will I have to go in for an exam? If an exam is needed, will a speculum be used (hopefully no!), or is that just for a pap?

2. Is there any real danger of not getting a period, or can this be ignored? How long should one wait before calling a dr?
