Menopause, perimenopause or what?

Two years ago I figured out I was getting violently ill the week before my period. DR put me on low does bcps. Still struggled with break through bleeding so he put me on a slightly higher dose. Now, I'm still having break through bleeding, crampig, bloating and back pain. Sometimes I have pain in the low pelvic region (not regularly enough to coincide with my cycle) which is sharp and breath taking. I'm 39 (only just) and had my first child at 34 and the second at 36. No more are planned. The pregnancies were uneventful and deliveries were normal. Supposedly the women in my family go through menopause early but my DR does not believe that is happening with me. I have been told the illness and such has to do with progestrone levels and perimenopause but no details were given. What should I be looking into to help me figure out what is going on?
