I am 50 years old with year long history of DUB. Endometrial biopsy,ultrasound found normal endometrium, enlarged uterus with heterogeneous echotexture suggestive of fibroids, no single focal mass, normal right ovary, left ovary with 2.3 cm cyst and a few follicles. At end of April, I was given Levlen twice daily to stop bleeding, went off and started bleeding on 4th day started pills on following Sunday. Bleeding started and stopped predictably using Levlen (which immediately perked me up both sexually and general "aliveness").

The good news is that I did stop and start bleeding as predicted and the (plus 3, according to hubby) pitting edema goes away while taking the Levlen.

The bad news is, bleeding is still heavy enough to limit activity. Also, on day 11 - 13 of the levlen, I had sharp, shooting pain in a quarter size area of lower left quadrant. Sharp, local pain on palpation. By day 12, hubby was going through differential for ED visit for either ruptured cyst or tortion of ovary but since it was intermittent and pain free between episodes, and timing was consistent with previous ovulation pain, we decided to wait and by day 13, pain was constant dull ache that peaked and disappeared.

My question is this: Did I ovulate even though I was taking Levlen or is the hormone causing the cyst found on ultrasound to become problematic and, 2: What do I do about the heavy bleeding - is it just hormonal or do I have a flock of tiny fibroids and this is not going to get better with medical management?
