GYN101 does a wonderful job of telling your wife what to expect, but does very little for the husband. From your prospective, the exam should approximate this process. She will be called back. She will be weighted and have her vital signs taken. She may be asked for an urine sample; if not, make her go before the exam starts, [inquire where is the bathroom]. Once in the exam room, she should be provided with a smock or gown (3 arm hole type, the last wrap goes across the front) and a drape. Assist your wife getting undressed and into the exam garb. Ask her where she would like to sit and wait on the examiner. The first part is just like any other exam; eyes, ears, nose, throat, stability, orientation, breath sounds and heart. Then the upper body will be exposed for a visual breast exam, upright palpation exam of the breasts and associated lymph nodes. Now, she will be asked to lay back and the slide will be pulled out for the legs to rest on. A two-handed exam starts at the head and precedes towards the draped area. Breasts, thorax and abdomen are fully examined for any indication of problems. Now the part that has put more than one significant other, father and mother on the floor. The examiner will assist your wife into the stirrups and the associated position for the rest of the exam. [Bring some thick calf high socks in case the stirrups are not padded already.] The nickname of awkward, embarrassing, open to the World and nothing hidden position, describes the trying to get in to it, the feeling of degradation and of being a piece of meat on display. There are normally three to five exams done. Pap Smear, External, Internal visual, internal structures (bi-manual) and rectal. Normally, the External is done first and is normally over so fast the patient does not even realize it was even done. Then a duck-bill speculum is introduced and opened. Cells are removed for pathological examination. As the speculum is removed, it is maneuvered to visualize the entire vault. Now the fingers of one hand are used to manipulate the internal organs while the other hand's fingers are feeling the internal organs. An RV exam is done here based upon indications and ditto for a rectal exam.
