Abdominal Cerclage

Hi. I'm 30 years old and am currently 11 weeks pregnant. I have just recently been told that it would be in my best interest to have an abdominal cerclage within the next week or two.In 1996 I lost a baby at 22 weeks gestation. Tests were done to show that I have an incompetent cervix as well as a malformed uterus. No further testing was done to find out what kind of malformation is there. Possibly a unicoronate, bicoronate, or deviated septum uterus. I got pregnant in 1998 and delivered my healthy son at 33 1/2 weeks gestation with the help of a McDonald and Shirodkar suture both put in place. I am told that my cervix is not long enough to perform the same vaginal surgery as last pregnancy. I have concerns about this new abdominal cerclage. Concerns for the baby as well as myself. Surgery is to last approx. 1 to 1 1/2 hours and they will put me under. The team of Doctors have given me a choice. If I decline abdominal surgery, they will do their best to input a suture vaginally but they are telling me the abdominal surgery would give me a better chance. Apparently the risk for infection for the baby is less with abdominal surgery than with vaginal surgery due to the fact that the abdominal surgery places a rubber band higher up on the uterus keeping the baby from moving close to the cervix throughout the whole pregnancy. The Doctor who performed my last surgery in 1998(shirodkar/mcdonald) informed me that I did not have alot of cervix he could work with but managed to input it anyway. I was not on any bedrest for that pregnancy in 1998 and managed to carry to 33 1/2 weeks. My question is could I have the vaginal cerclage now, stay in bed for 6 months and possibly carry closer to term? Or should I go ahead and let them perform abdominal surgery? Is one surgery better than the other? Has anyone died from abdominal cerclage? How common is abdominal cerclage? How many years has it been performed? I do not want to deliver at 33 weeks or less again. My goal is to make it to term. Please help with as much information as you may have.

Much thanks in advance. Nervously waiting.
