I hope you'll be pleasantly surprised. Because the speculum is smooth and can be lubricated with water for its insertion, it may be less uncomfortable than a tampon.

No need for that, but you might want to think about ways to induce relaxation during the exam -- prayer, visualization, breathing exercises, bringing a "talisman" of some sort, or having a friend present to distract you are all possibilities.

No, there's absolutely no need for that unless you are severely, severely constipated (I've only seen one case in which I couldn't do an exam due to constipation, and the issue was insertion of the speculum. The rectal exam would not have been a problem.)

You're obviously making some great health decisions by delaying intercourse and getting a gyn exam. Best of luck!
