This is most likely hormonal(anovulatory), and may stop with provera, altho with the fact you bleed so long, your md may want to try "birth control cascade" (3 pills a day for 7 days) before a d&c. It is possible it is anatomic, like a fibroid or polyp. It is good it is slowing with provera No clotting means it is not true 'period or withdrawl' bleeding. Good Luck.
