Post surgery problems- for Dr. von Almen

I could use your opinion on what to do next. Thanks again for your help!

I had basket surgery for a kidney stone in 1996.

In 1997, I had an ablation, myolysis and resection of fibroid tumors ( 20 week preg.size uterus)- turned into a supracervical hysterectomy and R salpingo-oophorectomy because of bleeding that could not be stopped during surgery. They found adenomyosis and several fibroids in all layers according to the Pathology Report. I also had light bleeding regularly each month until the ovary shut down 2 years later. Menopause was verified by blood levels of hormones. Prior to this time and after menopause I continued with bad cramps and pevic discomfort. My doc said I may have some endometrial tissue in the cervix and some remaining adenomyosis. I am on Cenestin, but have to take 300mg of Prometrium everyday to get rid of this discomfort. I worry about being on that much Prometrium because of the breast Ca risk. I have tried cutting back , but the cramps and cervical discomfort comes back. After this surgery I was having sharp pain in the R lower side down into the groin. An MRI suggested a mass on the vaginal cuff.

In 1999, I had a laparoscopic burch suspension. At that time they found the so called mass from the previous surgery was actually a hematoma and evacuated it. My R side felt so much better. I wound up going back to the OR the same day because of bleeding from the L port site. A 4" incision was made and a drain put in for a few days. I was black and blue up into the back on the L side. When the drain was pulled out, I felt a sharp pain from the lower L side , around the waist and into the back (kidney area). He cleaned up adhesions during this surgery.

My main problem is what to do next. This wrap around pain from the lower L side around the waist into the back contiues. Sometimes it is just lower, sometimes upper and sometimes all areas. Seems worse after stretching, exercise and after a bowel movement. They ruled out a kidney stone with the IVP. I went to PT- seemed to make things worse. Tried pain management. The swimming bothered me. Had me on Vioxx and Amytriptyline. He thought it was the ilioinguinal nerve. He injected it with Bupivicaine. He felt he couldn't help me.

I just use Darvocet as needed only when the pain is bad. It just takes the edge off it. I also have degenerative disc disease and degenerative arthritis. My GYN is looking at possibly doing another laparoscopy and opening the old drain incision to see if there is nerve entrapment.

What is your thought on all this?

Also, libido is good. My testosterone level is great. My arousal is not as good as it used to be. What is good for this?

Is 300 of Prometrium OK each day?

I just had a breast cyst removed from the aerola about 3 weeks ago. This was a small area ( looked like one of the little areas that stay up when chilled)- only this stayed up, got darker in color and was getting larger. The Path. Report said it was smooth muscle hyperplasia with papillomatosis. A note was made- considered in the microscopic differential diagnosis is an accessory nipple ( I am 50 yrs old- wouldn't I have noticed this awhile back? This area became sore to touch as it was growing this year.) Then it said- less likely smooth muscle hamartoma. Clinical correlation is recommended. What does this all mean?

I will be glad to get some help with this. Have just been taking this on step at a time. I am so grateful that I did not have anything major going on through all this and my doc has been wonderful. With sincere appreciation-Anne
