Biopsy is done most of the time whe a colposcopy is done. I like to simplify and say that Pap says something is wrong, Colpo says here is where it is at, and biopsy says this is what it is. In general the colposcopic signs of Invasive cancer are rather subtle. The less important ones stand out and hit you in the face, but the real important ones are subtle and then we don't see them very often, because invasive cancer of the cervix is really so rare. Prior to pap smear it was number one female cancer, now it is about number 35. The pap smear finds and then we treat precancer.
