Thank you for this, Terry...

I did not respond to this topic because I am tired of saying the same thing over and over.

15 minutes apart is obviously too short a time to assess hi-risk ob's the way we usually do them. However, when I worked in a MFM environment, this is just what we did. The doctor did not scan every patient because many patients were repeated and repeated and repeated. And repeated. The protocol was, measurements only if the last sono was 1 wk ago. Many, many BPP's w/AFI's. Anomalies got at least 2 scans, one with an amnio. BPP's twice a week. High risk ob is really active in the 3rd trimester, anyway. There were very few 2nd trimester exams in that department, except, of course, the amnios. What takes me the longest is the 2nd trimester Level II exams. The third trimester is much faster. Especially because they've been scanned so often. Anatomy was essential on all except BPP's.

Well, this was my experience, anyway. But it was a city hospital, where the patients didn't pay for any of the exams or have any insurance. I assume the doctor got paid, but I don't know how.

I had done ob's for many years before this experience, but never this kind of volume. The doctor did talk to the patients, but not for overly long sessions. But keep in mind that they came back once or twice a week very often, and had regular charts with us.

The quality of the pictures suffered, because that is what takes time, getting good quality pictures. But perinatologists are often very good as sonographers and can read even the slightly lesser quality pictures very very well.

I'm sure that in your case the doctor will be taking pictures while he scans. And he will give you the cassettes to develop (assuming you still use cassettes). Probably he is asking the department for an extra machine and room to make the flow go faster.

I know I would hate this situation....I tried being a waitress while I was in college, and I was a DISMAL failure. I got tips because the people felt sorry for me!!!

What do you MFM perinatologists think about 15 minutes per exam?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
