Reply to: RE> Medicare rules for sonograms Thanks to Diana Ross for the reference to the new Medicare rules. We don't do much Medicare Ob (a little in dependents, and CHAMPUS uses Medicare rules). Fortunately for us, Connecticut Medicaid does not use HCFA regulations. The proposal is obviously better overall for sonographers, but still can be abused by the physician who simply signs off on the sonographer's work and can't read an ultrasound to save his/her life without a sonographer there to turn the machine on. I am trying to set up a realtime transmission network for basic Ob sonography, and the rules as currently written would preclude that unless an MD is in the office, even though one of our faculty would be seeing the sonogram live. This is the same system that Malone & D'Alton have described in a couple of recent papers. I have asked Fred Kremkau for clarification of AIUM's position. My thought is that there should be an exception for realtime sonograms that are read "live" by the physician. Right now the AIUM Accreditation Commission is requiring that the physician be able to see the films/sonogram within 30 minutes as representing adequate supervision. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
