In screening patients we accept a clear abdominal view, but we have been very surprised in a few high risk patients by how wrong a "clear" abdominal view could be. I still cannot explain this.
The standard of care for all US has always been to Look - not necessarily to See. I expect the sonographers to look. Roberto Romero had a JUM editorial to that effect, and he went over the WHY much better than I can here..

You're checking the cervix on all patients, regardless of indications?
Vaginally? I hear the pot bubbling for sure.
