TVS is modality of choice for women who want to alter the basic risk of severe aneuploidy without amniocentesis (by NLT and fetal assessment at 12-14 weeks), women at risk for prematurity and (in our lab) for screening for preterm when cx not well seen by TAS (40% of the time).

IT ALSO HAS BEEN PROVEN BY H FRANK ANDERSEN, AND NOW CONFIRMED INDEPENDENTLY BY TWO OTHER GROUPS, TO BE MODALITY OF CHOICE TO ASSESS CX IN HIGH RISK PREGNANCIES. Have been using TVS for high risk assessment since 1983, when I looked for split hand foot at 12 weeks, and correctly excluded it. It is used after FFn with symptoms (PLEASE do not use FFn without symptoms!), and to follow high risk cx and previas and high risk hx of PTB and twins - in conjunction with vag pH and slide tests for BV.

I once had an auto mechanic who couldn't find an oil leak. I fired him when I found out he was looking in the glove box and trunk but didn't have time to get under the hood....... ;^) Jim
