An episiotomy is less likely to be needed with other than first children. It is difficult to tell if an epis will be necessary. I don't decide until the last minute. If there is likely to be a tear, I am more apt to cut an epis than to risk and extended tear. uestion your doc more carefully about the epis. I don't think that that issue would be problematic enough for you to consider leaving your doc, but certainly do what you feel comfortable with.
