I cannot say for sure if you have PCOS but based on your irregular periods (too many is irregular just like too few), weight gain, insulin problem and high testosterone, I would say that it is very likely that you have PCOS.

I had very thick hair but losing it for me that was one of the first symptoms. Most women have other problems first. Some women do not develop their symptoms until their mid to late 20's or 30's. Even other women develop worse symptoms after menapause. PCOS is a syndrome and as such, each of us have a different range of symptoms.

With your insulin resistance, low-carbing and met should have helped. The met takes 6-9 months in many women before they see a change. Most will start ovulating and have a period before then though. With the diet, you may have had "hidden" carbs or things that trigger an insulin release. Gum, mints, artificial sweeteners, etc. could have caused you to not loose weight. You also may not have had a plan that was suitable for you. One thing that is very important is water. The more I drank, the more I lost.

I do not undersatand why you are on spiro. if you have normal body hair and normal scalp hair.
